POSITION SUMMARY: The Agency Service Supervisor will assist the Agency Director in all administrative operations of StoneBridge, serve as supervisor for the companion, sitter, or respite service staff/contract, and will act as the authorized administrative representative of the Agency Director in his/her absence. The Registered Nurse Supervisor will be responsible for the supervision of the In-Home Aide.

DUTIES AND RESPONSIBILITIES:

1. Participates in coordinating scheduling assignments
2. Participates in orientation 
3. Ensures that services are coordinated
4. Encourages the appropriate use of community resources
5. Participates in recruitment, interviewing and hiring
6. Supervise the companion, sitter, respite staff by contacting the client receiving care every three months and by making a supervisory visit to each client's place of residence at least every six monthswith or without the companion, sitter, respite staff/contract’s presence, and at least annually, while the companion, sitter, respite staff/contract is in the home providing services to the client.
7. Review of the client's general condition, monitoring progress and response to the services provided by the companion, sitter, respite staff/contract and updates to the plan of care as needed.
8. Participates in, and completes, yearly performance evaluations
9. Participates in scheduled in-services
10. Performs disciplinary actions
    1. Is authorized, in the absence of the Agency Director, to be Acting Administrator in matters concerning the home care services.
11. Participates in Annual Program Evaluation
12. Participates in quarterly client casereview
13. Assures office operations 

QUALIFICATIONS:

1. Organizational Skills
2. Strong written and verbal communication skills
3. Minimum of one-yearmanagement or supervisory experience in home care

Job Description: The In-Home Aide will assist with limited-hands on care, companion, respite and home management tasks required by the clients.

5. Empty trash
6. Sweeping
7. Mopping
8. Vacuuming

Sitter and Respite Services include:

1. Providing long or short-term supervision and monitoring for the client in their home to ensure safety and well-being

EDUCATION, TRAINING, AND EXPERIENCE:

1. High School Diploma/GED
2. Good listener
3. Valid driver's license
    

POSITION SUMMARY: The agency Registered Nurse Supervisor will provide direct supervision of the Certified Nursing Assistant and In-Home Aide.
   
DUTIES AND RESPONSIBILITIES

1. Admit all clients to the agency
2. Develop plan of care for clients
3. Participate in the agency Nurse on -call system
4. Validate the skills of the Certified Nursing Assistant and In-Home Aide prior to assignments
5. Perform 90-day supervisory visits of the In-home Aide and Certified Nursing Assistant
6. Participate in hiring, promotion, and dismissal decisions
7. Assist in the development of in-services for the agency
8. Participate in quarterly record review for the agency
9. Other duties as assigned

 
EDUCATION, TRAINING, AND EXPERIENCE:

1. Valid Registered Nurse License
2. Valid North Carolina driver's license
3. Graduate from an accredited degree/diploma nursing program
4. At least 6 months experience in health field
